Referrals and Assessments

Referrals can be made by individuals themselves, family members, social workers, healthcare professionals or care managers. We have a straightforward referral process that begins with completing our referral form, available for download on our website or by contacting our team directly. Professional referrals should be sent to referrals@applenetcare.co.uk

Once we receive a referral, our assessment team will review the information provided and determine whether our services may be suitable. If so, we'll arrange an initial assessment meeting with the individual and, where appropriate, their family members or professionals involved in their care. This assessment helps us understand specific needs and whether we can provide appropriate support.

The time frame varies depending on the complexity of needs and the urgency of the situation. Typically, the process from initial referral to placement decision takes 2-4 weeks. In emergency situations, we can expedite this process where capacity allows.

If our assessment indicates that we cannot meet an individual's specific needs, we will provide clear reasons and, where possible, suggest alternative services or providers that might help.
